Application/ Policy Letter 08-09 : This form is to be used when you are enrolling a new student in the WSA for private instruction.  This must be completed in order for a skater to receive statement billing. YOU will not receive payment for lessons given until this form is turned in to WSA administrator, Shannon Felter.

Independant Contractor Agreement 2008- 09 : If you have not already done so for the 2008-09 skating season, PLEASE complete this form and give it to WSA Administrator, Shannon Felter.

Private Instruction Discount Coupons : Please see Sara Payment for details on how to use these coupons.  Please note that when using these coupons you must sign them and agree to giving this lesson at 25% off of your regular instruction rate.
